Document that shows the name and address of the hospital where the child was born. (such as hospital brochure, invoice, or printout of the hospital website.)

(Parents who are not British nationals) UK Immigration status when the child was born.

▶Where the parent has settled status, please also submit the old BRP card or letter/email from the Home Office that shows the date of acquisition of settled status.

Important Information

  1. If you post the documents, please do NOT submit the original passport, however a copy of the passport is required.
  2.  A Family Registry (Koseki Tohon or Koseki Shohon) is not required; however, you will need to be able to write your Registered Domicile (Honseki-chi) correctly. Please check the information beforehand. Do not abbreviate the Registered Domicile (Honseki-chi) to 「1-2」. Instead, write exactly as it shows on the Family Registry, such as「1丁目2番地」.
  3. Additional supporting documents may be required on a case-by-case basis.
  4. Submitted documents will not be returned.
