Important Information

1. The Declaration of Choice of Nationality forms must be written in Japanese.
   Write your name exactly as it appears on your Family Register. 
2. Additional supporting documents may be required on a case-by-case basis.
3. Please write your contact number on the bottom of the registration form.
4. If the person who makes the declaration of the choice of nationality is under 15 years of age, both parents, or a legal guardian must sign the registration form as the informants (See sample). If he/she is over 15 years of age, he/she must sign the registration form as the informant.
