Required Documents

2.

  Death Certificate issued by the Registry office

 1 Original and 1 photocopy
3.

  Translation of Death Certificate (Template別ウィンドウで開く/ Sample別ウィンドウで開く)

 2 sheets
4.

  Declaration for the time of Death (Template別ウィンドウで開く)

 2 sheets
5.

  The deceased person’s passport (The passport will be voided and returned.)

  Informant’s Photo ID

  
 
Who can submit the documents (the informant)?
  1. Relative of the concerned person
  2. A person living in the same household
  3. The landowner or caretaker of the house or the land
One of the above are required to submit the registration.
 

Important Information

  1. Additional supporting documents may be required on a case-by-case basis.
  2. Please write your contact number on the bottom of the registration form.
  3. If you post the documents, please do NOT submit the original passport, however a copy of the passport is required.
