Self-Assessment Medical Report

How do I fill it in?

This report is the MEDICAL tab of your application form and is completed in your application folder along with the rest of the form; the declaration you sign at the end of the FORM tab covers it. All applicants are required to complete the Self-Assessment Medical Report, detailing any health conditions that may require medical attention during their time in Japan. Some questions become required depending on your answers, and your application cannot be declared complete until they are answered.
 
 

Statement of Physician

Do I need one?

Depending on the information provided in the Self-Assessment Medical Report, you may also need to submit a Statement of Physician. This is usually required in the following circumstances:
  • You have received treatment or taken medication in the last five years.
  • You are currently taking medication or having treatment.
  • You have a long-term or recurring condition.
  • You have previously been treated for any nervous or mental conditions (including consultations).
If you declare any medical condition on the Self-Assessment Medical Report, you must provide a Statement of Physician.

How do I get one?

The Statement of Physician must be completed, stamped, and signed by hand by a qualified doctor with access to your medical history. This is often your GP. As your medical record in Japan will be limited to only what is provided through JET paperwork, your doctor must complete all sections in as much detail as possible, and make a clear assessment as to whether you are fit to participate on the JET Programme for a minimum of one year. Your doctor may continue onto a separate sheet if necessary. This form can be difficult for applicants to sort out on time, so we recommend contacting your GP early in the application process so that you meet the deadline.
 
 

Certificate of Health

Do I need one?

Early Departure applicants will need to submit the document at application time, whereas applicants for Summer Departure will need to obtain this document once they have received their application outcome.

How do I complete it?

Please have your Certificate of Health completed by a doctor in its entirety. Any missing information can delay the application process. The medical information outlined in the Certificate of Health should be consistent with your other medical forms (Self-Assessment Medical Report and/or Statement of Physician). You must provide the test results for the TB test (in most cases this is an x-ray imagining report or blood test) alongside the Certificate of Health. Medical conditions disclosed later in the application process will need to be explained further by the applicant (please see Personal Medical Statement below).

My GP doesn't provide X-rays/TB tests/eye tests, what should I do?

Most GPs cannot offer X-rays but can offer applicants referrals that can be used at hospitals or private institutions. In this case, the results must be signed off by a radiologist and/or your GP. In the same way, every part of your Certificate of Health must be signed off by a doctor. This means it is acceptable to have part of your Certificate of Health completed externally, as long as it is authenticated by a doctor and the results for that part of the form are provided.

I'm overseas, how can I complete my Certificate of Health?

If you are currently overseas, you do not need to return to the UK to complete your Certificate of Health. An overseas medical practice can complete this form for you. You will need to provide the current version of the document and the practice will need access to your medical history.

This document then needs to be translated into English, so please leave plenty of time to complete this part of the process.
 
 

Personal Medical Statement

Do I need one?

Most applicants will not need to provide one, unless requested. However if a candidate feels that a condition needs further explanation, they may provide one. A Personal Medical Statement may be requested in cases where a condition stated on an applicant’s Statement of Physician needs further elaboration or has been omitted in the earlier application stage.

This Personal Medical Statement should be typed and signed, explaining your condition, how it affects/doesn’t affect your daily life, the duration of the condition, any medications taken and how you currently cope with the condition.

Please note that applicants are expected to disclose all existing and previous medical conditions within their application. Missing information/documents during the application process can result in delays to processing or even disqualification.
